Requesting a Personal Sacramental Record
Sacramental records can only be provided to the individual whose name is in the sacramental notation or with the individual’s written permission. Information may be released to parents and guardians if the individual is under 18 or if there is legal documentation that entitles a person to request this information. If the individual is deceased a family member may access their sacramental record with the proper identification.
If you are searching for a personal sacramental record and you are unaware of the church location, please submit a request to the Archives using the form below, listing all known information about the sacrament’s place and time, i.e. Chatham, September 1970. Archives staff will assist you with directing your request to the proper parish office.
